What are all-on-6 dental implants?

All-on-6 replaces every tooth in one arch with a single fixed bridge that is screwed onto six dental implants. With more implants spread from the front towards the back of the jaw, the bridge has additional points of support, which may reduce the load placed on each implant and allow the bridge to extend further back with less cantilever.

The approach generally needs more bone along the length of the arch than All-on-4, because implants are placed further back where the upper sinus or the lower nerve canal can limit space. Some patients therefore need a bone graft or sinus lift beforehand, while others already have sufficient bone. The implants may be placed upright or with a modest tilt, depending on anatomy.

Like other full-arch concepts, treatment is usually staged: a provisional fixed bridge where stability allows, followed by a definitive bridge after integration. Choosing six implants rather than four is not automatically “better” – it is a planning decision based on your bone, bite and long-term maintenance needs.

What they can help with

  • Provide a stable, fixed replacement for a full arch of teeth
  • Distribute chewing forces over six implants rather than four
  • Shorten unsupported cantilever sections at the back of the bridge
  • Offer some redundancy, so the plan may adapt if a single implant runs into difficulty

Who are all-on-6 dental implants for?

The deciding factor is usually bone: there must be enough healthy bone in six suitable locations, or a realistic plan to create it. Your clinician will review a 3D scan before recommending the number of implants.

Often a good fit

  • Adults with no teeth, or failing teeth, in one or both jaws
  • Patients with adequate bone along the arch, or who are willing to consider grafting
  • People with heavier bites who may benefit from broader implant support
  • Denture wearers looking for a fixed full-arch option

Assessed carefully

  • Bone height and width in the back of the jaw, including sinus position in the upper arch
  • Gum health and any history of periodontal disease
  • Smoking, vaping and other habits that affect healing
  • Medical conditions or medicines affecting bone metabolism or bleeding
  • Clenching or grinding, and whether a night guard will be needed
  • Ability to clean around six implant sites and attend maintenance

Honest information

Benefits, limitations & risks

Potential benefits

  • More points of support, which may reduce stress on each individual implant
  • Shorter cantilevers at the back of the bridge, which may lower the risk of bridge fracture
  • Some built-in redundancy if one implant needs attention
  • A fixed arch that stays in place during eating and speaking

Limitations

  • Requires more bone than four-implant designs, so grafting may lengthen the overall timeline
  • More implants mean more surgical sites and more areas to keep clean
  • Not every jaw can accommodate six well-spaced implants
  • The bridge remains a prosthesis that may need maintenance, repair or eventual replacement

Possible risks

  • Post-operative swelling, bruising and tenderness
  • Infection, wound breakdown or slower healing, especially where grafting has been performed
  • Implant failure to integrate, which may require a revised plan
  • Peri-implantitis if plaque control or maintenance is inadequate
  • Sinus or nerve complications when implants are placed further back, minimised but not removed by careful planning
  • Chipping or fracture of the bridge and loosening of screws over time

The procedure

All-on-6 Dental Implants, step by step

The aim is to position six implants where bone is healthiest and where they give the bridge the most even support. Planning considers bone, bite and how easily you will be able to clean around each site.

  1. Step 01

    Consultation and CBCT scan

A cone-beam CT scan, photographs and digital impressions are used to measure bone along the full arch and to decide whether six implant positions are achievable without, or with, grafting.

  2. Step 02

    Preparatory surgery if required

Where bone at the back of the jaw is insufficient, a bone graft or sinus lift may be carried out first and allowed to mature for some months before implants are placed.

  3. Step 03

    Extractions and implant placement

Remaining teeth are removed if necessary, the ridge is prepared and six implants are positioned along the arch according to the digital plan, often with abutments attached to align the bridge screws.

  4. Step 04

    Provisional bridge

When the implants show good initial stability, a provisional fixed bridge may be connected soon after surgery. A soft-food period protects the implants while they integrate.

  5. Step 05

    Definitive bridge

After healing, updated scans guide the design of the final bridge. A try-in lets the team check fit, bite, speech and appearance before the bridge is screwed into place.

  6. Step 06

    Long-term reviews

Regular check-ups, X-rays where indicated and periodic professional cleaning with the bridge removed help protect the implants and surrounding gum.

How All-on-6 differs from All-on-4 and implant overdentures

Both All-on-6 and All-on-4 provide a fixed bridge screwed to implants. All-on-4 relies on four implants with tilted rear fixtures to work around limited posterior bone, whereas All-on-6 adds two further implants for broader support, which usually needs more bone or grafting. An implant-supported denture is removable by the wearer, typically uses fewer implants and may rest partly on the gum.

Before & after treatment

Preparation & aftercare

Before you travel

  1. Provide details of any medications, especially blood thinners or bone-related treatments
  2. Have any gum infection or decay in remaining teeth assessed before planning
  3. Reduce or stop smoking ahead of surgery if possible
  4. Ask how grafting, if needed, would affect the number and timing of visits
  5. Prepare soft, nutritious meals for the early healing period

Looking after them

  1. Your clinician’s personalised instructions come first; this is general guidance only
  2. Follow a soft diet until you are told the implants can take normal chewing
  3. Manage swelling with cold packs and recommended pain relief
  4. Keep the surgical areas clean with gentle rinses as directed, then add brushes and water flossers when advised
  5. Wear a night guard if one is prescribed for grinding
  6. Keep every review and professional cleaning appointment

Questions

Asked about all-on-6 dental implants.

General answers — your own plan always depends on an individual assessment.

All dental questions
Is All-on-6 stronger than All-on-4?

Six implants may spread forces more widely, but strength depends on bone quality, implant positions and bridge design as well as numbers. Your clinician recommends the layout that fits your anatomy rather than a set number.

Why might I need a sinus lift for All-on-6?

In the upper jaw the sinus often sits close to the bone at the back of the mouth. Placing additional implants there can require more bone height, which a sinus lift may create.

Does treatment take longer than All-on-4?

It can, particularly if grafting is needed first. If bone is already sufficient, the timeline may be similar.

Will I have teeth while the implants heal?

Where implant stability allows, a provisional fixed bridge is often fitted early. Otherwise a temporary denture may be used during healing.

How do I clean an All-on-6 bridge?

You brush the bridge as you would teeth and clean underneath it with tools such as water flossers, interdental brushes or special floss. Your hygienist will show you techniques suited to your bridge.

Can I have All-on-6 through VERAKS HEALTH?

If this option is relevant to you, availability at our clinic and your suitability are confirmed during your consultation.
