What is wisdom tooth extraction?

Wisdom teeth, or third molars, are the last teeth to develop and usually emerge – if at all – in the late teens or twenties. Many jaws do not have enough space for them, so they may remain fully or partly trapped beneath the gum or bone. This is called impaction, and a wisdom tooth can be impacted upright, tilted forwards, tilted backwards or lying horizontally.

Not every wisdom tooth needs to be removed. Healthy, fully erupted wisdom teeth that can be cleaned may be monitored. Extraction is generally considered when a wisdom tooth causes or is likely to cause problems: repeated gum infection around a partly erupted crown (pericoronitis), decay that cannot be treated, damage to the neighbouring molar, cysts, or pain.

A fully erupted wisdom tooth can often be removed much like any other tooth. An impacted tooth may need a surgical extraction, in which the gum is opened, a little bone may be removed and the tooth is sometimes divided into sections so it can be lifted out with less force.

What it can help with

  • Resolve recurrent infection, pain or swelling linked to a wisdom tooth
  • Protect the adjacent second molar from decay or root damage
  • Remove a source of cysts or other pathology seen on X-ray
  • Make the back of the mouth easier to clean

Who is wisdom tooth extraction for?

The decision to remove a wisdom tooth is based on symptoms, clinical findings and X-rays – not on age alone. Your clinician will weigh the benefits of removal against the surgical risks for your particular tooth.

Often a good fit

  • People with painful, infected or partly erupted wisdom teeth
  • Those whose wisdom tooth is damaging or trapping food against the next molar
  • Patients with a wisdom tooth that is decayed and cannot be restored
  • Anyone with X-ray evidence of a cyst or other change around the tooth

Assessed carefully

  • The angle and depth of impaction
  • Proximity of lower roots to the inferior alveolar nerve, or upper roots to the sinus
  • Root shape, number and curvature
  • Age, as bone tends to become denser and healing slower over time
  • Medical conditions and medicines affecting bleeding or healing
  • Current infection, which may need to settle before surgery

Honest information

Benefits, limitations & risks

Potential benefits

  • May end recurring infection and pain from a problematic wisdom tooth
  • Can protect the neighbouring molar from further damage
  • Removes a hard-to-clean area at the back of the mouth
  • Addresses pathology such as cysts identified on imaging

Limitations

  • Swelling and restricted mouth opening are common for several days
  • Removal of a symptom-free wisdom tooth is not always recommended
  • Some extractions are complex and may need referral or a longer appointment
  • Healing of the socket and bone continues for weeks after the soft tissue closes

Possible risks

  • Dry socket, where the clot is lost and the socket becomes painful
  • Swelling, bruising and jaw stiffness
  • Infection of the socket
  • Temporary or, rarely, lasting numbness of the lip, chin or tongue if nerves are affected
  • Communication with the sinus when removing some upper wisdom teeth
  • Bleeding, particularly in people taking anticoagulants

The procedure

Wisdom Tooth Extraction, step by step

Planning centres on the tooth’s position and its relationship to nerves and the sinus, so the least traumatic method can be selected.

  1. Step 01

    Examination and X-rays

A panoramic X-ray is commonly taken. If the roots appear close to the nerve canal or sinus, a cone-beam CT scan may be recommended to see their relationship in three dimensions.

  2. Step 02

    Discussing the plan and consent

Your clinician explains whether a simple or surgical extraction is expected, the specific risks for your tooth and your anaesthesia options, so you can make an informed decision.

  3. Step 03

    Anaesthesia

The area is numbed with local anaesthetic. Sedation may be offered for anxious patients or more complex extractions, depending on what is available and appropriate.

  4. Step 04

    Removing the tooth

For an erupted tooth, it is gently loosened and lifted out. For an impacted tooth, a small gum flap is raised, some bone may be removed and the tooth may be sectioned into pieces to minimise trauma to the surrounding tissue.

  5. Step 05

    Cleaning and closure

The socket is cleaned, any sharp bone edges are smoothed and stitches are placed if needed. Gauze is applied so a blood clot can form, which is essential for healing.

  6. Step 06

    Recovery and review

You receive written aftercare advice. A review may be arranged to check healing or remove non-dissolving stitches.

Preparation & aftercare

Before you travel

  1. Share your medical history and medicines, especially blood thinners
  2. Eat a light meal beforehand unless you are told to fast for sedation
  3. Arrange for someone to accompany you home if you have sedation
  4. Stock up on soft foods and plan a few quiet days afterwards

Afterwards

  1. Your clinician’s personalised instructions take priority over these general points
  2. Bite on gauze as directed and avoid disturbing the clot for the first day
  3. Do not smoke, use straws or rinse vigorously in the first days
  4. Use cold compresses early on, then warm salt-water rinses when advised
  5. Eat soft, lukewarm food and chew on the other side
  6. Seek advice if pain worsens after a few days, or if bleeding or swelling increases

Questions

Asked about wisdom tooth extraction.

General answers — your own plan always depends on an individual assessment.

All dental questions
Do all wisdom teeth need to be removed?

No. Healthy wisdom teeth that are fully erupted and easy to clean may be kept and monitored. Removal is considered when there is a clear clinical reason.

How long will the swelling last?

Swelling commonly peaks around the second or third day and then settles over about a week, though this varies with the complexity of the extraction.

What is a dry socket?

It occurs when the blood clot in the socket is lost early, exposing bone and causing aching pain a few days after extraction. Your clinic can dress the socket to ease symptoms.

Can all four wisdom teeth be removed at once?

Sometimes, depending on their positions, your health and anaesthesia options. Your clinician may also suggest removing them in stages.

When can I travel after the procedure?

Many people feel able to travel within a few days, but it is sensible to allow time for a review. Your clinician can advise based on your extraction.

Is wisdom tooth extraction available through VERAKS HEALTH?

If this treatment is relevant to you, availability at our clinic and your suitability are confirmed during your consultation.
