What are implant-supported dentures?

An implant-supported denture – also called an implant overdenture – is a full denture that attaches to dental implants placed in the jaw. Instead of relying only on suction and the shape of the gums, the denture clips onto attachments on the implants, which can greatly reduce movement during eating and speaking.

Several attachment types exist. Individual stud or ball-style attachments connect the denture to each implant, while a bar design links the implants with a metal bar onto which the denture clips. The choice depends on the number of implants, jaw shape, space available and how much support the gum should still provide.

In the lower jaw, two implants are frequently enough to stabilise an overdenture considerably. The upper jaw usually needs more implants because the bone is softer. Unlike fixed full-arch bridges, you remove the denture yourself to clean both it and the implants.

What they can help with

  • Improve denture stability and confidence when eating and speaking
  • Reduce sore spots caused by a moving denture
  • Help preserve jawbone around the implant sites
  • Offer an implant option with fewer implants than many fixed designs

Who are implant-supported dentures for?

Overdentures may suit many denture wearers, including some who are not candidates for fixed implant bridges. A 3D scan and examination confirm whether implants can be placed safely.

Often a good fit

  • People without teeth in one or both jaws
  • Current denture wearers frustrated by movement or poor grip
  • Patients with moderate bone loss who may not suit a fixed bridge without grafting
  • Those who prefer a prosthesis they can remove and clean themselves

Assessed carefully

  • Bone volume where implants would be placed
  • Space between the jaws for attachments and denture thickness
  • Smoking, diabetes and other factors affecting healing
  • Hand dexterity for inserting and removing the denture
  • Commitment to cleaning and attending maintenance

Honest information

Benefits, limitations & risks

Potential benefits

  • Often much more stable than a conventional denture
  • May allow more comfortable chewing of a wider range of foods
  • Typically needs fewer implants than fixed full-arch bridges
  • Removable, making cleaning and repairs relatively straightforward
  • Upper overdentures with enough implants can sometimes be designed with reduced palate coverage

Limitations

  • Still a removable denture, which some people find less natural than fixed teeth
  • Attachments and clips wear and need regular replacement
  • The denture base can still put some pressure on the gums
  • Requires surgery and a healing period before final attachment

Possible risks

  • Swelling, discomfort and bruising after implant surgery
  • Infection or failure of an implant to integrate
  • Peri-implantitis if implants are not kept clean
  • Denture fracture or wear of attachments
  • Sore spots while adjusting to the new denture

The procedure

Implant-Supported Dentures, step by step

The aim is to combine the simplicity of a removable denture with the stability that implants provide, choosing the attachment design to suit your mouth.

  1. Step 01

    Assessment and scan

Your existing denture, gums and bone are assessed and a cone-beam CT scan is taken. A well-fitting denture is often used to plan implant positions.

  2. Step 02

    Implant placement

Two or more implants are placed under local anaesthesia. Your denture may be adjusted and relined with soft material so you can keep wearing it while healing.

  3. Step 03

    Integration

The implants bond with the bone over several months. In some cases, early loading may be possible if stability is high.

  4. Step 04

    Attachment connection

Abutments or a bar are connected to the implants, and the matching housings are processed into a new or existing denture.

  5. Step 05

    Fitting and instruction

You practise inserting and removing the denture, the bite is adjusted and you are shown how to clean the attachments and implants.

  6. Step 06

    Ongoing maintenance

Retention inserts wear over time and are replaced periodically, and the denture may need relining as the gums change.

Implant-supported dentures vs All-on-4 and All-on-6

An implant-supported denture is removable: you clip it on and take it out to clean, and it often uses two to four implants with the gum sharing some of the load. All-on-4 and All-on-6 are fixed bridges screwed onto implants, which only your clinician can remove; they usually require more implants and different bone conditions, and feel closer to natural fixed teeth.

Before & after treatment

Preparation & aftercare

Before you travel

  1. Bring your current dentures to every appointment
  2. Share your full medical history and medications
  3. Stop smoking before surgery if you can
  4. Plan for soft foods in the first days after implant placement

Looking after them

  1. Your clinician’s personalised instructions take priority over this general guidance
  2. Follow advice on whether to wear your denture immediately after surgery
  3. Remove and clean the denture daily, and clean around each implant or bar carefully
  4. Soak the denture as recommended and handle it over a towel or water to avoid breakage
  5. Report loose attachments or sore spots promptly
  6. Attend regular reviews for insert replacement and relining

Questions

Asked about implant-supported dentures.

General answers — your own plan always depends on an individual assessment.

All dental questions
How many implants do I need?

In the lower jaw, two implants are often used, while the upper jaw commonly needs more. Your clinician bases this on bone quality and the denture design.

Can my existing denture be used?

Sometimes, if it fits well and is in good condition. Often a new denture is recommended so the attachments can be incorporated precisely.

Do I sleep with it in?

Many clinicians advise removing dentures at night to rest the gums and clean the attachments, but follow the advice you are given.

How often do the clips need replacing?

Retention inserts wear with use, so periodic replacement is normal. Your clinician will check them at reviews.

Is it better than a fixed bridge?

Neither is universally better. Overdentures can need fewer implants and are easier to clean; fixed bridges feel more like natural teeth. The right choice depends on your anatomy and priorities.
