What is moustache transplant?

A moustache transplant moves individual hair follicles, most often from the back of the scalp, into the upper lip area to create, thicken or even out a moustache. It is a small-area procedure, but a detailed one: the skin of the upper lip is mobile and sensitive, the hairs need to lie almost flat against it, and small differences in direction are easy to see in such a central part of the face.

Design comes first. The shape is drawn with you, considering your lip, the width of your mouth, the corners where the moustache meets the beard and the style you would like to wear. Some people want to fill one or two gaps; others want a complete moustache where little hair grows. The number of grafts depends on that design, on how much hair you already have and on the density you are aiming for, so it can only be estimated as a range after you have been examined.

Grafts are usually single-hair follicular units, chosen because they look finer and more natural in facial skin. They are placed at very shallow angles, following the outward and downward direction in which moustache hair naturally grows from the midline. Where scalp hair is much finer or softer than your existing facial hair, your clinician may discuss beard hair from under the chin as a donor source, if it is dense enough. Transplanted hair keeps some of the characteristics of its donor site, so its texture may differ slightly from native moustache hair.

What it can help with

  • Create or fill a moustache where growth is sparse, patchy or absent
  • Even out asymmetry between the left and right sides
  • Connect the moustache naturally with the beard at the corners of the mouth
  • Camouflage a scar on the upper lip where the skin is suitable

Who is moustache transplant for?

A moustache transplant may be considered by adults whose facial hair pattern has settled and who have suitable donor hair. Skin conditions, scarring and the reasons for missing hair are assessed first.

Often a good fit

  • Adults with sparse or patchy moustache growth and a settled facial hair pattern
  • People with good donor hair that is similar in calibre to facial hair
  • Those with healthy skin on the upper lip and no active inflammation
  • Patients with realistic expectations about density and texture

Assessed carefully

  • Donor hair calibre, colour and curl compared with your facial hair
  • Causes of hair loss such as alopecia areata, which may need medical assessment
  • Scar tissue, which can have a less reliable blood supply
  • Skin conditions, acne or a tendency to raised scarring
  • Smoking and medical conditions that affect healing
  • Whether the moustache is part of a larger beard plan using the same donor supply

Honest information

Benefits, limitations & risks

Potential benefits

  • Uses your own hair, which continues to grow and can be trimmed and styled
  • Can be tailored precisely to the shape you want
  • Small treatment area with a relatively short procedure
  • Can be combined with beard work in the same plan

Limitations

  • Texture or colour may differ slightly from your native facial hair
  • More than one session may be needed for the density you want
  • Grafts placed in scar tissue may grow less reliably
  • Donor hair used here is not available for future scalp or beard work

Possible risks

  • Swelling, redness and tenderness of the upper lip for several days
  • Small crusts, spots or ingrown hairs as grafts grow
  • Temporary numbness of the lip or donor area
  • Hairs growing at an unnatural angle, which may need adjustment
  • Uneven or lower-than-expected growth
  • Infection or visible marks, which are uncommon

The procedure

Moustache Transplant, step by step

A moustache sits at the centre of the face, so the plan concentrates on proportion, direction and fine grafts rather than graft numbers.

  1. Step 01

    Design consultation

The shape and extent of the moustache are discussed and drawn in front of a mirror. Your clinician explains how the design relates to your face, how many grafts it is likely to need as a range, and which donor area is most suitable.

  2. Step 02

    Donor preparation

A small donor area, usually at the back of the scalp, is trimmed and numbed. Fine single-hair grafts are extracted with a micro-punch, in an even pattern so the donor area is not visibly thinned.

  3. Step 03

    Recipient sites

The upper lip is numbed. Very small sites are created at shallow angles that follow the natural flow from the centre outwards, slightly more upright near the lip edge and flatter towards the sides.

  4. Step 04

    Graft placement

Grafts are placed one by one, with spacing and direction checked against the design. The procedure is usually shorter than a scalp transplant, but it is meticulous work.

  5. Step 05

    Healing and growth

Small crusts form around each graft and fall away over the following days. Transplanted hairs often shed within the first weeks, and new growth usually begins to appear after a few months.

Moustache transplant or beard transplant?

Both use the same principles and often the same donor area. A moustache transplant concentrates on the upper lip, where the skin moves constantly and the hairs must lie very flat in a fan-like pattern from the centre. A beard transplant covers cheeks, chin, jaw and neck and usually needs more grafts. Many plans combine the two, in which case donor supply is planned for the whole face.

Before & after treatment

Preparation & aftercare

Before you travel

  1. Avoid shaving or trimming the moustache area for the period advised so existing hair can be seen during design
  2. Tell your clinician about medicines, skin conditions and any history of cold sores
  3. Avoid alcohol and medicines that increase bleeding before surgery, if advised
  4. Plan a few quiet days afterwards, as the upper lip is visible while it heals

Afterwards

  1. Follow your written instructions; they take priority over this summary
  2. Keep the upper lip clean as shown and avoid rubbing or picking at crusts
  3. Eat soft foods and drink carefully in the first days to avoid pulling at grafts
  4. Do not shave the moustache area until your clinician says it is safe, often after a few weeks
  5. Avoid saunas, swimming and strong sun for the advised period
  6. Report increasing redness, swelling or discharge

Questions

Asked about moustache transplant.

General answers — your own plan always depends on an individual assessment.

All hair questions
How many grafts does a moustache transplant need?

It depends entirely on the design, how much hair you already have and the density you want. A few gaps need far fewer grafts than a full moustache. Your clinician gives an estimated range after examining you.

When can I shave or trim after the procedure?

Shaving is usually avoided for a few weeks while grafts anchor and the skin heals. Your clinician will tell you when it is safe and how to trim carefully at first.

Will the transplanted hair look like moustache hair?

It can blend well, especially when single-hair grafts are angled correctly. Hair from the scalp may be slightly softer or straighter than native facial hair, which is one reason donor choice is discussed at assessment.

Can a moustache transplant cover a scar?

Sometimes. Scar tissue may have a weaker blood supply, so growth can be less predictable and a second session may be needed. The scar is assessed before this is recommended.

Will the transplanted hairs fall out?

Shedding within the first weeks is common and expected. The follicles usually remain beneath the skin, and new growth typically begins after a few months.
